This session starts with presenting the most impactful evidence-based interventions used in the Million Hearts® initiative and approaches to implementation that consider health equity, resources, and local factors to help optimize hypertension and cholesterol outcomes in your health center(s). We will then move to focused peer-to-peer conversations to help you curate practical tips, ideas, and resources to formulate an action plan to bring back to your organization.
