Amy Alward, BSN, RN, PCMH CCE, serves as the Director of Population Health at the Michigan Primary Care Association (MPCA). She has been with MPCA since November 2020. Amy received her bachelor’s degree in 2002 from the Michigan State University College of Nursing. Her role involves providing direct support and technical assistance to Community Health Centers for their participation in clinical initiatives and grants. Specifically, she oversees and implements activities related to the prevention, management, and treatment of chronic diseases such as Diabetes and Hypertension. Amy also supports the implementation and use of population health technology to achieve clinical outcomes. As a Patient-Centered Medical Home Certified Content Expert (PCMH CCE), she provides technical assistance to MPCA’s network of Community Health Centers in meeting and maintaining NCQA Patient-Centered Medical Home recognition.
