Older adults are the fastest growing segment of the U.S. population as well as within health centers. Are your health center systems and care models designed to care for the aging population? Has your health center explored the intersection of age and health equity? Is your health center putting in place systems to attract and retain Medicare patients, create an age-friendly environment, and be the provider of choice in your community? This interactive workshop will provide actionable steps your health center can take to design care models, implement necessary infrastructure, recruit and retain the workforce to care for this segment of the patient population and develop partnerships with key community-based organizations. Panelists will provide insights and recommended strategies that can be adopted by health centers across the country.
