This abstract aims to provide an overview of key topics that will be covered in our upcoming presentation, focusing on the integration of pharmacy and specialty care services, community-based mobilization through value-based community agreements, and the vital role of community health workers all in realizing financial, equity, and community perspectives.
Health equity is a fundamental principle that underpins the mission of FQHCs. Our presentation will delve into the importance of addressing disparities in healthcare access, outcomes, and resources for underserved populations. By exploring strategies to integrate pharmacy and specialty care services, we will examine how FQHCs can enhance comprehensive care delivery, improve patient outcomes, and optimize resource utilization. We will highlight successful case studies that demonstrate the positive impact of these integrated services on health equity.
Furthermore, we will delve into the concept of value-based community agreements, which foster collaborative partnerships between FQHCs, community organizations, and stakeholders. These agreements enable a holistic approach to care by addressing social determinants of health and engaging community resources. We will explore the benefits and challenges of implementing value-based community agreements, and provide practical guidance on how to navigate these partnerships effectively.
Recognizing the crucial role of community health workers, we will discuss their unique contributions in bridging the gap between healthcare providers and underserved communities. We will examine the strategies for recruiting, training, and deploying community health workers to enhance patient engagement, improve health outcomes, and ensure that the perspectives of diverse communities are represented and realized in healthcare decision-making processes.
In summary, this presentation will provide a comprehensive overview of health equity as it relates to FQHCs and advancing value-based care. By exploring the integration of pharmacy and specialty care services, value-based community agreements, and the role of community health workers, we aim to equip CEOs and Medical Directors with actionable insights and strategies to promote health equity within their organizations. Together, we can drive positive change and ensure that all individuals, regardless of their background, have access to high-quality, equitable healthcare services.
