CMF1 - Partners of Choice: CDC Directors Panel: Aligning Public Health and Health Center Priorities

The integration of public health and primary care and taking action together could be the key to improving health outcomes, reducing health inequities, and protecting the health of communities across the nation. This session will provide the opportunity to hear from CDC leaders as they discuss the shared priorities between health centers, NACHC and the Centers:
1. Electronic case reporting: Data integration and standardization:
2. Addressing low vaccination rates: Improve health centers' ability to support and deliver immunizations.
3. Hypertension control: Bringing public health and CHC best practices to bear on health disparities around hypertension.
4. Youth mental health: amplify school mental health action guide and other school-based behavioral health efforts.

Panelists will share their insights on advancing these foundational components of public health-primary care integration, including bi-directional sharing of data, identifying aligned priorities and making shared investments, and working toward payment models that cover community-based prevention programs and reward population-level outcomes. Attendees will have the opportunity to engage with thought leaders to identify priority collaboration opportunities, including outlining the important role health centers play in achieving shared successes.
