Care Coordinator III
Native American Health Center
Ellen McInnes (she/her/hers) began her career as a Community Health Worker at the Native American Health Center (NAHC) in San Francisco. Her work as a CHW has centered around direct patient care, creating organization specific workflows, collaborating with other community based organizations, and completing a year long CHW certificate program with support from the San Francisco Community Clinic Consortium. Currently at NAHC, she is a care coordinator for patients who are part of the CalAIM Enhanced Care Management program. Ellen received her Bachelor’s degree in Latin American Studies and Spanish and completed a Health Studies certificate from Dickinson College in 2022. During undergrad she undertook a thesis project centered on indigenous health and community activism in Ecuador. Ellen was born in Ecuador of indigenous Andean heritage and spent her childhood in a small rural town in western New York. Both circumstances have influenced her work academically and professionally to focus on the areas of health equity, education, and advocacy for all her patients. She strives to uphold cultural humility while providing care to Native American, indigenous, and Latinx communities in San Francisco’s Mission District.
